Dr. Emily Nelson’s last day to see patients in the clinic is August 28, 2026. Patients with appointments after this time will be contacted by our care team to transfer your care to another provider. Patient medical records will remain on file with the practice. Patients who would like a copy of their medical record should download a request form from the Patient Center. Please fax the form to (512) 930-4946 or mail it to 3201 S. Austin Ave. Suite 210 Georgetown, TX 78626. For assistance in obtaining a medical record, please call (512) 763-4000.
